Welcome to Mekks Home Health Care. We believe those needing home health care should get the very best. Our goal is to provide quality personalized care through appropriate coordination of services to meet your specific healthcare needs. Under the direction of your physician, our team of qualified health care professionals will provide on-going case management, if needed that is appropriate, competent, and consistent.
Assessment & Evaluation
Nursing visits will consist of a complete physical assessment and evaluation of the patient. The skilled nurse will also instruct the patient and family/caregiver in disease process and management, medications & other pertinent topics. Usually nursing visits will be frequent at first. Then as acute medical problems are resolved and home instruction completed, visit frequencies decrease and discharge planning and teaching will begin.
